How to use it

  1. Read each question with the words “Over the last 2 weeks, how often have you been bothered by…”.
  2. Choose not at all (0), several days (1), more than half the days (2) or nearly every day (3).
  3. Read the total, the severity band, whether it reaches 10 and the GAD-2 line. Your answers stay in your browser.

How the score works

The seven answers are added together, so the GAD-7 runs from 0 to 21.

Total Severity
0–4 Minimal
5–9 Mild
10–14 Moderate
15–21 Severe

The GAD-2 is the sum of questions 1 and 2 (0–6); 3 or more is positive.

Worked example

Answers 2, 2, 2, 2, 1, 1, 0 give 2 + 2 + 2 + 2 + 1 + 1 = 10, moderate, which reaches the cut-off of 10. The GAD-2 is 2 + 2 = 4, positive.

Tips

  • Think about the last two weeks as a whole, not just a bad day.
  • Use the same questionnaire again after a few weeks to see whether things are changing.
  • Answers of “nearly every day” on several questions matter even if the total is just under 10.
  • This page is informational only. If anxiety is affecting your life, talk to a doctor.

Frequently asked questions

What do GAD-7 scores mean?

0–4 is minimal anxiety, 5–9 mild, 10–14 moderate and 15–21 severe. A score of 10 or more is the cut-off most often used to suggest a fuller assessment for an anxiety disorder.

Does the GAD-7 only detect generalised anxiety disorder?

It was developed for generalised anxiety disorder, but higher scores are also common with panic disorder, social anxiety and post-traumatic stress. The questionnaire cannot tell these apart; a professional assessment can.

What is the GAD-2?

The first two GAD-7 questions: feeling nervous or on edge, and not being able to stop worrying. Each scores 0–3, so the GAD-2 is 0–6, and 3 or more is a positive screen that makes the full questionnaire worthwhile.

Is a high score a diagnosis?

No. The score measures how often symptoms bothered you in the last two weeks. Thyroid problems, caffeine, some medicines and stressful events can raise it too. Only a doctor or another qualified professional can diagnose an anxiety disorder.
